Participation Requirements

PHYSICAL FORMS


Each student athlete is required to have an annual physical. The physical is required for students who wish to tryout and/or participate in any sport. The physical must be current (dated April 15 or after) for the current school year.

There are two additional forms which are explained below, that are now required by State Legislation (Concussion form & Sudden Cardiac Arrest form).

Concussion Forms
Effective January 1, 2014, TSSAA requires that all schools and community organizations sponsoring youth athletic activities must establish guidelines to inform and educate coaches, youth athletes, and other adults involved in youth athletics about the nature, risk, and symptoms of concussion/head injury. Therefore, all active participants of AMS Athletics (players, parents, and coaches) must have a Student-athlete & Parent/Legal Guardian Concussion Statement on file before they can actively participate in athletic activities.

Sudden Cardiac Arrest Forms
Effective January 1, 2016, TSSAA and State legislation requires that all schools and community organizations sponsoring youth athletic activities must establish guidelines to inform and educate coaches, youth athletes, and other adults involved in youth athletics about the nature, risk, and symptoms of sudden cardiac arrest. Therefore, all active participants of AMS Athletics (players, parents, and coaches) must have a Student-athlete & Parent/Legal Guardian Cardiac Arrest Statement on file before they can actively participate in athletic activities.

ACADEMIC & BEHAVIORAL REQUIREMENTS

In order to participate in a sports team at AMS, student athletes must maintain a "C" average in all classes including MAPS and not receive any Ns or Us in conduct.
